Request a Birth or Death Certificate
The Fishers Health Department provides birth and death records from April 24, 2020, to now. For records before this, contact:

Certificates are available via certified/USPS mail or pick-up:
- Location: Vital Records Office, 3 Municipal Drive, Fishers, IN 46038
- Hours: Monday – Friday, 9 a.m. – 4:15 p.m.

Request a Birth Certificate

- Online requests are monitored Monday through Friday.
- You will receive email notification when your request is approved, usually within 1-2 business days.
Request a Death Certificate
- Request online or in-person.
