Request a Birth or Death Certificate
The Fishers Health Department provides birth and death records for anyone born or who has died in Indiana from 1907 to present day. For records before 1907, please contact:

Certificates are available via certified/USPS mail or pick-up:
- Location: Vital Records Office, 3 Municipal Drive, Fishers, IN 46038
- Hours: Monday – Friday, 9 a.m. – 4:15 p.m.

Request a Birth Certificate

- Request online or in-person.
