The Fishers Health Department is one of the few in Indiana enrolled in the FDA’s Retail Program Standards program, which ensures inspections focus on areas with the highest impact on foodborne illness prevention. This shifts the focus by making the department a public health program with a regulatory component rather than a regulatory program with a public health component.
Apply for a permit for a food facility, Farmers’ Market vendor, Home-Based vendor, or Mobile Food vendor. All food vendors within the City of Fishers must obtain and display an active permit issued by the Fishers Health Department.
The FDA provides timely updates on food recalls, safety alerts, and public health notifications to help you stay aware of potential foodborne hazards. For comprehensive and current information, visit the FDA’s Food Safety Alerts page to find detailed alerts and advice on how to handle affected products.
The ByHeart Inc. Whole Nutrition Infant Formula (24 oz cans and Anywhere Pack™) is being voluntarily recalled nationwide in coordination with the U.S. Food & Drug Administration (FDA). The recall is being expanded due to concerns about potential contamination with Infant botulism-causing *Clostridium botulinum spores or toxin.
What you should do:
If you have purchased this product, stop using it immediately and dispose of any remaining product.
If your infant is experiencing any of the following symptoms — constipation, poor feeding, drooping eyelids, weak cry, difficulty swallowing or breathing — seek medical attention right away.
To report an illness or adverse event related to this formula, contact the FDA at 1-888-INFO-FDA (1-888-463-6332) or complete the FDA’s Voluntary MedWatch form online.
Why this matters: Infant botulism is a rare but serious condition that can result when C. botulinum spores are ingested by an infant and produce toxin in the immature gut. Although no unopened ByHeart product has yet tested positive for C. botulinum spores or toxin, ByHeart and the FDA are acting out of an abundance of caution.

A Permanent Retail Food Establishment is an operation that stores, prepares, packages, serves, vends, or otherwise provides food for human consumption. The term includes all operations where consumption is on or off the premises, and regardless of whether there is a charge for the food. These are a few examples: a restaurant, a catering operation, a grocery store, a tavern, or a food bank.
A Temporary Retail Food Establishment Permit is required for a food establishment that operates at a fixed location for a temporary period. Permits are available in a variety of time increments for convenience and reduction of cost to the vendor. Temporary events may include city celebrations, fairs, and festivals.
A Food Truck Permit is required for any mobile concessions operation within City limits. These permits are offered in a variety of time-based increments to meet the needs of our vendors.
